PRISM risk score.
Enter age, sex, karyotype and mutations. See the PRISM-3 risk group (Low, Moderate or High) for newly diagnosed AML treated with a hypomethylating agent plus venetoclax, and exactly how the score is built.

Lachowiez et al., J Clin Oncol 2026. An independent implementation; the authors’ own calculator is at prism-aml.com.
How it works
- Enter age, sex at birth and karyotype, then tick every gene reported as mutated. Anything unticked is scored as not mutated.
- The PRISM score is a sum: a weight for each finding, added to a weight for each year of age. There is no intercept. “How the score is built” lists every term, biggest first.
- The PRISM-3 group compares the score with two cut points: Low is below 1.0988, Moderate is from there to below 1.6132, High is 1.6132 or more.
- “Decisive findings” shows which findings the group hinges on. Click one to see what happens without it.
- The 4-gene group is a separate, simpler grouping shown beside it: TP53 mutated is Adverse; otherwise FLT3-ITD, KRAS or NRAS mutated is Intermediate; otherwise Favorable.

Source and limits
- Implements the PRISM model of Lachowiez et al., J Clin Oncol 2026;44(25):2422–2434. An independent implementation, not affiliated with or endorsed by the authors. If this page and their calculator ever disagree, trust theirs.
- The coefficients, cut points and input ranges are those of the authors’ online calculator. This page was checked against it on 22 October 2026: identical scores on more than 20,000 inputs, including values either side of each cut point.
- For adults aged 18 to 100 with newly diagnosed AML treated with a hypomethylating agent plus venetoclax. It gives a risk group, not a survival estimate or a treatment recommendation.
- A reference tool for healthcare professionals. It does not diagnose, recommend treatment, or replace clinical judgment. Do not enter patient identifiers; none are needed.
